Thanks W, for doing this. Difficult to give advice in a condition like this, with all the meds and all..

-Could be hypo T on the bad days. The medication may cause this; I see it in all hypo T medicated collegues and friends, one day good, the next day very bad. Then their doctor adjust their meds, more up and downs.

A good indication of a good day is the temp; 37 C in average. Monitor his actual temperature; to see if there is a connection. Hypo T`s can also have spells of heat, like in the morning. Then they are cold for the rest of the day.
Or opposite; cold in the morning, warmer throughout the day.

-yes and no; magnesium is used for fluoride detox.
He may however not utilize the magnesium he takes; change type, amount.

Boron; YES he needs it. Balance the magnesium, cramping etc.
A big must from GG, tell him:)
Also the tightness of the diahpragm, could be magnesium deficiency.

"Any suggestions on nerve regeneration?"
V mentioned Lithium.
Also vit B1 in large doses; or preferably benfotiamine;

B12 helps too. Vit B 6 for magnesium uptake. Not talking RDA here; therapeutic dosages needed.
Vitamin B`s also needed for medication handling. All water based vitamins are used en masse in all kinds of illness, adrenals; need vit C. And I bet his adrenals are in a bad shape. Trauma.
