Your case sounds like so many others, which is a good thing because many have already been successful on similar journeys.
With the large amounts of Antibiotics , you are definitely experiencing dysbiosis, which is when the intestinal flora ratios become unbalanced. This condition can account for every single one of your symptoms, so the goal is to reverse your dysbiotic condition.
The way to do this is the get the yeast levels down by a strict diet at first while rotating powerful antifungals like oregano and coconut oil. I also recommend h202 (hydrogen peroxide) inhalation. There are many others as well but these are the ones that come to my mind.
You also need to make sure your body's pH level is at the best level for good intestinal flora to grow. This is easily done using organic unfiltered apple cider vinegar.
Molybdenum can help with the brain fogginess for some. Others may have suggestions for this as well.
Once you get the yeast down, you can start to add some fruits, which are very healing to the body, plus they taste great.
As to CS, I have not used it personally but I understand you can use it indefinitely.
In terms of die-off, feeling bad is not always a sign of the herxeimer effect. So for this reason, until you are very far along on your journey, you will probably have multiple periods where you don't feel well. These periods will repressent both die-off and symptoms of your condition. As you get better, these periods will become less frequent, until eventually they go away completely.
