I'll take the first question. Yes, the vegetables need to be raw when you juice them. Cooking kills off the enzymes, which are why you are juiciing in the first place. You should then strain the juice so that all you are drinking is the liquid. I am not sure about adding water, as it may oxidize the juice, which is what you don't want, but my understanding is that you want to avoid dairy products, as they have too much protein, which slows down the fast.
