Most home colonic systems are based on gravity to introduce the contents of the bucket into the colon. The bucket can be filled by a variety of methods, from manually pouring pitchers into it until it's full, to a professional plumbing option where a waterline is installed to automatically fill the bucket.

Essentially, home colonic systems work the same way as suspending an enema bag or can and laying there as the bag or can empties. What the expensive home colonics offer is a board to make eliminating easier, and the ability to take more than a single enema during a session without having to get up and go to the toilet in between enemas.
