I'm guessing the crawling sensation is more so strongylodies, not pinworm. Strongylodies take years to eradicate. I'd follow dr. Yu's protocol, but keep going after on a solid monthly worming schedule. (A solid one, like Sharkmans). I'm on 9months now, and realize this is a very long term disease.
Wash all produce thoroughly. American produce is teeming with autoinfective parasites. Best of luck
