The reason for the ground flax seed is because it's mucilaginous when wet, and that helps hold it all together...hence the recommendation of ground flax, slippery elm or IF#2 (the IF #2 contains SEVERAL ingredients that are mucilaginous...slippery elm, marshmallow, flax and the psyllium)

The final consistency of the poultice should be about like toothpaste.

Correct, you sandwich the 'black toothpaste goo' between the dry paper towels, and none of it touches your skin...nor do you want it to touch your skin (charcoal will permanently "tattoo" even the slightest skin break), and you need something airtight/waterproof on top of it to prevent the charcoal from adsorbing 'the air' (and all the toxins in it).
