The only painless (excepting a possible 24 hours of vomiting) and very effective way to cure a serious addiction, as far as I've read, is with the root bark of the African plant Iboga, or the laboratory-synthesized chemical Ibogaine HCl, which is alledgedly the main active ingredient of the plant. It's banned in America (surprised?) and a few European countries, but is available throughout most of the world. It's fairly expensive: hundreds of dollars for a dose of the root bark, many hundreds for the refined chemical, and thousands if you want to be supervised in a treatment clinic. Plus the cost of traveling to another country if you're in America. But reportedly, it only needs to be taken once to completely lose the desire to use most hard drugs for months, and with very little withdrawal. Then with some positive lifestyle changes, quitting forever becomes much easier. More info is online.
Pardon me for going somewhat off topic. Although fasting would probably do an especially good job of curing the addiction, I think the heroin withdrawal would still be a nightmare, so he would need to be 500% motivated to go through with it. Giving up food is difficult enough...
