The problem is that people will only accept help/teaching etc if they ask for it. You cannot help someone who doesn't think they need help. Or don't want it.
And there seems to be something "fashionable" about being sick. I find that people who, for instance, are on thyroid meds, feel kind of smug about it. Diabetes is not so cool, but some diseases seem to make people feel better about themselves (yeah, right). Statins are still hugely popular over here and they are also a status symbol. Expensive tests are too, and more so if they are invasive and require sedation!
