 my understanding is that lipolysis/ketosis takes at least 48 hours to truly kick-in, so how could that even happen on this kind of diet?

Again it depends on the individual. In a type 1 diabetic or alcoholic for example it should not take that long. And as I said earlier a lot of people often have underlying conditions they are not aware of. We see it all the time here on CZ. People guessing as to what they have, but not really knowing for sure.
