I have read your message, and you are in a situation that many people face, real dental problems and health problems that began at birth, during childhood, gestation, or are due to genetic influences.
I have dealt with some such issues myself and have had good results. I am not aware of other programs to deal with this, and I'm sure there are such programs, yet I have written a program for it.

To answer your question, "Where do I start?", I would say that year 1 is about assessment and education, without massive alterations to lifestyle. After once has learned about the problems and the methods available to address them, more weighty changes can be made with confidence.
The other important thing at the outset is to identify critical issues and neutralise any infections, tumours, or other forms of unwanted growth. This is done by local and general means.
Local strategies include creams or ointments and, once the infection is stopped, some very gentle local tissue massage. General strategies include nutritional support for cleaner blood, and low key exercise such as walking, or, in cold climates, relaxation techniques.
